There are plenty of things one may have forgotten ore one have been done wrongly.
- checking mesh's normals
- unconnected, overlapping points in a mesh
- overlapping objects
- wrong priority of a volume
- object at absolute position (0,0,0,)
- unchecked properties, like e.g. light tracing
- missing object volume, i.e. plane mesh instead of thin cube mesh
- cylinders smooth shaded without normals' option set to auto smooth
- et cetera
